Pre-admission Screening and Resident Review<\/span><\/p>

Camino Real Community Services provides services through the PASRR program to individuals who are seeking placement to a nursing facility or reside in a nursing facility (NF). To qualify for PASRR services an individual must have a diagnosis of an intellectual disability prior to the age of 18; a related condition prior to the age of 22; and\/or a mental illness without a primary diagnosis of dementia.<\/span><\/p>

PASRR services include:<\/span>
\n\u2022 Habilitation Coordination<\/span>
\n\u2022 Independent Living Skills<\/span>
\n\u2022 Day Habilitation<\/span>
\n\u2022 Supported Employment<\/span>
\n\u2022 Employment Assistance<\/span>
\n\u2022 Behavioral Support<\/span>
\n\u2022 Alternate Placement<\/span>
\n\u2022 Specialized Occupational therapy<\/span>
\n\u2022 Specialized Physical therapy<\/span>
\n\u2022 Specialized Speech therapy<\/span>
\n\u2022 Customized Manual Wheelchair<\/span>
\n\u2022 Durable Medical Equipment\n<\/span>
\nThrough PASRR services, individuals diagnosed with an intellectual or developmental disability or related condition, will have an assigned habilitation coordinator who will help ensure that individuals are receiving the required specialized services requested, have access to services, explore opportunities to live in a least restrictive setting, assist with transitioning from the nursing facilities to the community and receive meaningful and quality care. For individuals living in the community who meet the criteria for eligibility and is at risk of moving into a nursing facility PASRR can assist with a Diversion Slot which allows the individual to remain in the community through Home and Community Based Services.
