The following information describes the HCS and (TxHmL) programs and the services available to individuals enrolled with in these programs through Camino Real:\n\nHome &amp; Community Based Services (HCS) \u2013 a Medicaid wavier program which enables individuals to live in the community by providing needed supports and services. The program offers different residential options such as living in an apartment, companion care, or in the family home. It can provide assistance with respite care, supported employment, transportation, day habilitation, and service coordination.<br ><br >Texas Home Living (TxHmL) \u2013 a Medicaid waiver program which provides community-based services and supports to individuals who live in their own homes or in their family home. The program can provide assistance with community support, day habilitation, employment assistance, respite and specialized services. \n<br >\u2022 Individuals enrolled in the TxHmL program have access to the same services included under the HCS program with the exception of Host Home services or any other supervised living setting. \n<br >\u2022 The TxHmL program has a funding limit per individual set at $17,000 that they cannot go over. \n<br ><br >For information contact Clarisa Rodriguez at<\/span><br ><br >\n<\/h3>\n\n<\/div>\n<\/div>\n<div  class=\"row spaced-cols\" data-type=\"row\">\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">\n\n\nDay Habilitation Services<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">We provide individuals with assistance to help develop and refine skills necessary to live and work in the community. These services are provided in five different locations throughout the counties served by Camino Real Community Services.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Transportation Services<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Transportation is provided to the individual between home and the individual\u2019s community employment site or day habilitation site; and transportation to facilitate the individual\u2019s employment opportunities and participation in community activities.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Employment Services<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Services include teaching a person the skills needed to perform the job, to maintain the job, and to advance into other jobs.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Nursing<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Nursing services are offered to an individual who requires treatment and monitoring of health care procedures.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Respite<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Respite is either planned or emergency relief that is provided to the individual\u2019s unpaid caregiver on a short-term basis when the caregiver is temporarily unavailable.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Specialized Therapies<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Assessment and treatment by licensed or certified professionals for social work services, occupational therapy, physical therapy, speech and language therapy, audiology services, dietary services, and behavioral support.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">CFC Personal Assistance Services\/Habilitation<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Provides assistance with activities of daily living through hands-on assistance, supervision and training as desires. This service includes tasks such as eating, toileting, grooming, dressing, bathing, and transferring, meal preparation, shopping and household chores.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Behavioral Support<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Specialized interventions by professionals with required credentials to assist an individual to increase adaptive behaviors and to replace or modify maladaptive behavior that prevent or interfere with the individual\u2019s inclusion in home and family life or community inclusion.<\/span><\/p>\n\n<\/div>\n<div  class=\"col col-sm-6 space-bottom col-md-4\">\n<h4  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Dental Services<\/span><\/h4>\n<p  class=\"\"><span  style=\"font-family: arial, helvetica, sans-serif;\">Provides the individual up to $2000 per year for the cost of dental services.
