LIDDA is also responsible for coordinating and monitoring on-going services for an individual by assigning them a Service Coordinator. The service coordinator assists individuals with accessing medical, social, educational and other appropriate services and supports that will help the individuals achieve a quality of life and community participation acceptable to the individual based on their unique goals and preferences. LIDDA also maintains the HCS and TxHmL interest lists for persons residing in our 9-county service area, which includes: Atascosa, Dimmit, Frio, Karnes, LaSalle, Maverick, McMullen, Wilson and Zavala counties. Other important responsibilities include placing individuals on the Home and Community-Based Services (HCS) and Texas Home Living Interest Lists, helping students transition from school services to community-based services, and assisting families who are seeking residential services for children and adults. The LIDDA uses a Person-Centered planning process to develop an individualized plan of services and supports that incorporated trauma informed care and cultural sensitivity. This allows the individual and the family to direct the development of a plan that meets the individual’s outcomes.
