Pre-admission Screening and Resident Review

Camino Real Community Services provides services through the PASRR program to individuals who are seeking placement to a nursing facility or reside in a nursing facility (NF). To qualify for PASRR services an individual must have a diagnosis of an intellectual disability prior to the age of 18; a related condition prior to the age of 22; and/or a mental illness without a primary diagnosis of dementia.

PASRR services include:
• Habilitation Coordination
• Independent Living Skills
• Day Habilitation
• Supported Employment
• Employment Assistance
• Behavioral Support
• Alternate Placement
• Specialized Occupational therapy
• Specialized Physical therapy
• Specialized Speech therapy
• Customized Manual Wheelchair
• Durable Medical Equipment
Through PASRR services, individuals diagnosed with an intellectual or developmental disability or related condition, will have an assigned habilitation coordinator who will help ensure that individuals are receiving the required specialized services requested, have access to services, explore opportunities to live in a least restrictive setting, assist with transitioning from the nursing facilities to the community and receive meaningful and quality care. For individuals living in the community who meet the criteria for eligibility and is at risk of moving into a nursing facility PASRR can assist with a Diversion Slot which allows the individual to remain in the community through Home and Community Based Services.
